Job summary

StoneBridge Senior Living in Jacksonville, IL is seeking a registered nurse (RN) to supervise and coordinate nursing care for residents of our long-term care community. The RN will work under the Director of Nursing, ensuring compassionate, patient-centered care and adherence to regulatory standards.

The role emphasizes supervisory responsibilities, medication administration, care planning, and collaboration with CNAs/CMTs to meet resident needs. Strong communication and leadership are required.

Qualifications

• Must have a current state license to practice as a registered nurse (RN).
• Current CPR certification and additional certification in nursing specialty desired.
• Prefer 2 years’ experience in a long-term care setting.
• Basic computer skills and experience with Electronic Medical Records Preferred.
• Excellent communication and interpersonal skills.
• Ability to read, analyze and interpret professional journals, procedures or governmental regulations.
• Ability to define and solve problems, collect data, establish facts and draw valid conclusions.

Responsibilities

• Supervises nursing care provided by nursing assistants and others for whom they are administratively or professionally responsible.
• Plans, assigns, and directs work of nursing team members assigned (CNAs/CMTs/etc.).
• Prepares work assignment sheets, considering resident safety, education, experience, knowledge and ability.
• Reports staff shortages to nursing management.
• Assists to prepare performance evaluations with the understanding such evaluations impact tenure and wage increases.
• Schedules and adjusts breaks according to resident needs and labor laws.
• Receives and helps resolves employee complaints.
• Monitors and corrects job performance of employees.
• Coordinates nursing care of residents on the unit with appropriate nurse aides.
• Provides safe and accurate medication-related interventions and administers medications per schedule.
• Performs nursing assessments and notifies physician of changes in status.
• Utilizes INTERACT program to avoid unplanned hospital discharges.
• Develops a plan of care and implements nursing care.
• Follows facility policies and procedures.
