Job summary

HumanGood seeks a Resident Services Coordinator to organize, implement and evaluate the Social Services program, addressing psycho-social needs and resident independence. You will assess residents, maintain records, and link them to essential services.

Collaboration with the community team ensures confidentiality and a supportive living environment. The role requires 1+ year in case management, 2+ years social services in senior care, and a Bachelor's in social work or related field.

Qualifications

• Bachelor’s Degree in social work or related program such as gerontology, psychology, or recreation therapy
• 1+ years of experience in case management required
• 2+ years of social services experience, preferably in a senior care setting
• Bilingual - English/Mandarin
• Social Services certification preferred

Responsibilities

• Organize, implement and evaluate the Social Services program to meet residents' psycho-social needs
• Resident assessment and maintenance of resident records
• Ensure residents are linked to supportive services to continue living independently in the community
• Promote a positive living experience for residents
• Promote confidentiality and HIPAA privacy standards
• Collaborate with community team members to create a supportive environment
