Job summary

HumanGood in Dresher, PA, is seeking a Social Services Coordinator to organize, implement, and evaluate the Social Services program for residents. You will assess needs, maintain records, and connect residents with services to support independent living in the community.

The role requires 1+ year in case management, 2+ years in social services (preferably in senior care), and a Bachelor's in social work or related field; certification is a plus.

Qualifications

• 1+ years of experience in case management.
• 2+ years of social services experience, preferably in a senior care setting.
• Bachelor's Degree required in social work or related program such as gerontology, psychology, or recreation therapy.
• Social Services certification preferred

Responsibilities

• Organize, implement and evaluate the Social Services program to meet residents' psycho-social needs.
• Perform resident assessments and maintain resident records.
• Link residents to specific supportive services to help them live independently in the community.
