Presbyterian Homes & Services in Minnesota seeks a Household Coordinator to lead the Household team, partnering with the Clinical Coordinator to ensure high quality, person-centered care and meaningful activities for residents. You will supervise Resident Assistants, manage staff engagement, support daily living activities, and participate in care conferences.

Strong leadership and communication, plus knowledge of elder care regulations, are essential.

Qualifikationen

• Three to five years in older adult services; geriatric-related education/licensure preferred.
• Leadership to develop and motivate staff; oversee resource use and workflows.
• Ability to facilitate a self-led work team and develop facilitation skills.
• Competencies in quality management, leadership, resident and family support systems, budgeting.
• Strong interpersonal, problem solving, adaptability and communication skills.
• Nursing assistant training completion within six months of hire and current state certification.
• Compatibility with PHS mission and operating philosophies.
• Proficiency in reading, writing, speaking English for communication with customers.
• Excellent interpersonal, communication, and organizational skills.
• Proficiency with Microsoft Office and other common programs.

• Serve as resident advocate to achieve highest, purposeful self and preserve independence.
• Oversee quality of life, privacy, dignity, meaningful activity, and social needs.
• Implement PHS Liberty components in respective Household.
• Ensure Household staff engagement and daily living participation.
• Collaborate with supporting teams and volunteers to meet psycho-social and environmental needs.
• Facilitate weekly Quality Assurance meetings and Resident Council meetings.
• Provide leadership for Care Conferences and ensure regulatory compliance.
• Complete required MDS and charting documentation per regulations.
