Sentara Careplex Hospital in Hampton, VA is seeking an experienced Senior Inpatient Case Manager RN for the day shift. The role emphasizes leadership in daily inpatient case management and coordination of care across teams.

You will work within a 224-bed acute care facility with on‑site responsibilities, on‑call commitments including holidays and weekends. Virginia licensure and CM certification are required or pursued.

Qualifications

• RN with a BSN, MSN, or Doctorate as required.
• Virginia RN license (compact/multi‑state or single state).
• BLS certification required within one year of eligibility.
• Case management certification (ACM/CCM/CCCTM/CMAC/CMGT‑BC) within one year.
• Minimum 3 years of nursing experience; 3 years in case management preferred.
• Healthcare experience in an acute care hospital setting.

• Lead daily inpatient case management operations and coordinate work for the team.
• Assist supervisor and participate in team activities and case reviews.
• Ensure quality and service while maintaining fiscal accountability.
• Provide feedback on hiring and evaluations of personnel.

Overview

Sentara Careplex Hospital – Hampton, VA is hiring an experienced Senior Inpatient Case Manager RN to focus on complex cases.

Full Time – Day Shift, 40 hours per week. Hours: Mon‑Fri, 8:00 AM to 4:30 PM with on‑call commitments to include holidays and weekends.

Responsibilities

Assumes responsibility, accountability and leadership for the daily operations including coordination of work, quality and service for inpatient case management. Assists supervisor for assigned site/function. Participates in work activities of assigned teams and provides case management services as needed. Maintains fiscal accountability for departmental activities. Provides feedback on hiring and evaluations of personnel. Functions in an inpatient practice setting.

Qualifications

• Education: RN – Bachelor’s, Master’s or Doctorate degree.
• Licensure: Registered Nurse (RN) License—Compact/Multi‑State or Single State – Virginia.
• Certification: Basic Life Support (BLS) – American Heart Association; Case Management certification (ACM, CCM, CCCTM, CMAC or CMGT‑BC) required within one year of eligibility.
• Experience: 3 years of nursing experience (required); 3 years of case management experience (preferred).
• Health‑care experience in an acute care hospital setting.
