• Bachelor’s degree in Nursing (BSN)
• Current Registered Nurse (RN) license—Virginia or Compact/Multi‑State license
• BLS certification from American Heart Association (required within 90 days of hire)
• Specialty certification within one year of eligibility (ACM, CCM, CCCTM, CMAC, CMGT-BC)
• At least 3 years of case management experience
• Previous supervisory experience preferred

Responsibilities

• Supervise the day-to-day workflow and performance of the case management team across two hospital locations
• Ensure effective discharge planning and seamless coordination of inpatient care
• Support length-of-stay management, patient flow, and related performance improvement initiatives
• Provide coaching and feedback to staff to foster growth
• Address employee relations matters with leadership and HR
• Maintain compliance with department standards and hospital policies

Position Summary

As Case Management Supervisor, you will work across both hospitals, partnering with the Manager and Senior teams to ensure high-quality patient care, effective discharge planning, and efficient case management practices. Your expertise will be vital in supporting staff development and maintaining compliance, while leading a cohesive team focused on achieving exceptional outcomes.

Key Responsibilities

• Supervise the day-to-day workflow and performance of the case management team at both hospital locations
• Ensure effective discharge planning and seamless coordination of inpatient care
• Support length-of-stay management, patient flow processes, and related performance improvement initiatives
• Provide coaching, mentoring, and constructive feedback to staff to foster professional growth
• Address employee relations matters in partnership with leadership and HR
• Maintain compliance with departmental standards, hospital policies, and regulatory requirements

Requirements

• Bachelor’s degree in Nursing (BSN)
• Current Registered Nurse (RN) license—either Virginia or Compact/Multi‑State Basic Life Support (BLS) certification from the American Heart Association (required within 90 days of hire)
• Specialty certification required within one year of eligibility (acceptable certifications include ACM, CCM, CCCTM, CMAC, or CMGT-BC)
• At least 3 years of case management experience (required)
• Previous supervisory experience (preferred)
