Job summary

Sentara Careplex Hospital in Hampton, VA is seeking an experienced Senior Inpatient Case Manager RN to oversee complex inpatient case management and ensure high-quality patient care. This full-time day-shift role requires on-site leadership and collaboration with multidisciplinary teams.

The ideal candidate holds an RN with a bachelor’s, master’s, or doctoral degree, multi-state licensure or Virginia license, BLS, and 3+ years of nursing plus case management experience.

Qualifications

• RN license with multi-state or compact license.
• BLS certification required.
• 3 years nursing experience and 3 years case management preferred.

Responsibilities

• Lead inpatient case management operations on a daily basis.
• Coordinate care, ensure quality and service for inpatient cases.
• Provide feedback on staffing and evaluations within the department.

40 hours/week

Hours

• Mon-Fri - Hours: -8:00 4:30 PM with on-call commitments to include holidays and weekends

Education

• RN-Bachelor's Degree OR
• RN-Master's Degree OR
• RN-Doctorate Degree

Certification/Licensure

• Registered Nurse (RN) License - Compact/Multi-State License OR
• Registered Nurse (RN) License - Single State – Virginia
• Basic Life Support (BLS) - Certification - American Heart Association (AHA)
• Case Management (ACM, CCM, CCCTM, CMAC or CMGT-BC) certification required within one year of eligibility

Experience

• 3 years of nursing experience (required)
• 3 years Case Management experience (preferred)

Healthcare experience refers to the care delivered in one of the following settings: hospital or an acute care facility

Senior Inpatient Case Manager, RN: Assumes responsibility, accountability and leadership for the daily operations including coordination of work, quality, and service for inpatient case management. Assist supervisor for assigned site/function. Participates in the work activities of assigned teams and provides case management services as needed. Maintains fiscal accountability for departmental activities. Provides feedback on hiring and evaluations of personnel. Functions in one of the following practice settings: Hospitals only
