Job summary

Volunteers of America Colorado is seeking a Service Coordinator to oversee housing-related services for residents aged 62+ at Rendezvous Apartments in Montrose, CO. The role includes needs assessments, outreach, case management, and coordinating services to support aging in place.

The position is part-time (20 hours/week), with flexible hours, and requires collaboration with the Property Manager and local providers to improve resident wellbeing and access to benefits.

Qualifications

• Bachelor’s degree in human services, social work, or related field.
• At least one year of experience with seniors or people with disabilities.
• Willing to obtain CPR/First Aid certification and keep current.
• Criminal and civil background checks within 30 days of hire.
• Excellent interpersonal and organizational skills.
• Proficient in using Windows environment and working with databases.

Responsibilities

• Assess residents' needs and coordinate services to enable independent living.
• Conduct outreach, document interactions, and maintain records.
• Coordinate and monitor services with providers and care teams.
• Develop support groups and host workshops.
• Maintain resource guides and partnerships.
