VOAC is seeking a Service Coordinator to oversee housing-related services for seniors and people with disabilities in Colorado’s affordable housing portfolio. The role emphasizes needs-based assessments, community outreach, case management, and housing retention with trauma-informed care and motivational interviewing approaches.

Qualifications

• Bachelor’s degree in human services, social work, or a closely related field.
• At minimum, one year of experience with senior citizens and/or people with disabilities.
• Willingness to obtain first aid, AED, and CPR certifications and keep current.
• Criminal and civil background checks within 30 days of hire.
• Excellent interpersonal and organizational skills.
• Proficient with Windows and databases.

Responsibilities

• Ensure residents' voices are heard and incorporated into programming.
• Facilitate referrals from Community Administrator, family, providers, and residents.
• Assess resident needs via surveys, visits, and meetings; document information and eligibility.
• Coordinate services using HUD-aligned modalities and equity lens.
• Inform residents about services and providers; offer multiple options.
• Encourage residents to arrange needed services to maintain independence.
• Monitor provider services for quality and cost effectiveness.
