Job summary

VOAC is seeking a Service Coordinator at Westgate Towers Apartments in Denver. The role focuses on service provision and housing assistance for seniors and disabled residents, using Trauma Informed Care, Harm Reduction, and Motivational Interviewing.

You will conduct needs assessments, provide case management, support housing retention, and connect residents to income resources. Responsibilities include coordinating services, maintaining records, and developing resident programs.

Qualifications

• Bachelor’s degree in human services, social work, or a closely related field or related experience.
• At minimum, one full year of working with senior citizens and/or people with disabilities.
• Willing to obtain first aid, automated external defibrillator and CPR certifications and keep current.
• Criminal and civil background checks within 30 days of hire.
• Excellent interpersonal skills and good organizational skills.
• Proficient in using Windows environment and working with databases.

Responsibilities

• Ensure the resident’s voice is heard and their ideas influence programming.
• Facilitate referrals from Community Administrator, family, service providers and residents.
• Assess services needed by residents through surveys, visits and meetings; document assessments.
• Coordinate and monitor services from providers to ensure quality of care and cost effectiveness.
• Maintain up-to-date records on residents and services received.
• Develop and facilitate groups/workshops to meet resident needs.
• Implement the Service Coordinator program per HUD and VOA guidelines.
• Attend trainings, meetings, and conferences as required.
