Job summary

VOAC Colorado seeks a Service Coordinator to manage service provision and housing for residents in affordable aging/disabled housing sites. You will conduct needs assessments, outreach, and case management while coordinating care with providers to enable independent living.

Responsibilities include documenting interactions, maintaining records, and developing programs, with travel to sites as needed. This full-time role offers benefits and a path to impactful community work.

Qualifications

• Bachelor's degree in human services, social work, or closely related field.
• At least one year working with seniors or people with disabilities.
• Willing to obtain first aid/CPR certifications and keep current.
• Background checks required within 30 days of hire.

Responsibilities

• Assess resident needs through surveys and meetings and document information.
• Coordinate and monitor services with providers to ensure quality of care.
• Maintain up-to-date records and ensure Fair Housing compliance.
• Assist in relocating residents and supporting housing retention.
