Job summary

SSSEVA is seeking a Housing Stabilization Case Manager to provide housing-focused support for older adults facing instability. You will assess needs, coordinate services, and guide participants to trauma-informed, culturally appropriate resources.

Role emphasizes strong communication, HIPAA compliance, and collaboration with the Homeless Older Adults Manager. Primary daytime hours with occasional evenings or weekends, as approved.

Qualifications

• Bachelor’s degree in social work, human services, or related field.
• Minimum of 1 year of experience in community-based services.
• Excellent customer service and telephonic communication skills.
• Must possess a valid driver’s license, auto insurance, a good driving record, and pass a background check.

Responsibilities

• Facilitate person-centered, problem-solving conversations to identify participant needs and goals.
• Provide individualized, housing-focused support for older adult program participants.
• Complete strength-based housing barriers assessments and develop service/housing plans.
• Coordinate referrals to other providers and support participants in following through.
• Monitor and evaluate participants’ progress toward goals.
• Collect and input required data into HMIS and other platforms.
• Engage with local Continuum of Care planning efforts and elder coalitions.
• Support community outreach and program evaluation as needed.
• Collaborate with internal teams to ensure holistic support and smooth operations.
