Job summary

Partnership for Families, Children and Adults in Chattanooga seeks an Elder Housing Liaison to provide intake, assessment, and social services for individuals and families served. The role travels citywide to meet clients in homes, at partner sites, and in the community to complete intakes, gather and verify documentation, assess needs, and determine next steps.

Qualifications

• Bachelor’s degree in social work, human services, sociology, psychology or related field from accredited university required OR equivalent and relevant experience of 4 years or greater.
• Minimum of two (2) years of experience providing social services, case management, or client intake and assessment to individuals, older adults, and families required.
• Experience with housing support, home repair, homeownership, or financial stability programs preferred but not required.
• Knowledge of housing stability, financial capability, and community resources available in the Chattanooga/Hamilton County area.
• Knowledge of trauma-informed care and strengths-based case management practices.

Responsibilities

• Provide intake, assessment, and social services to individuals and families served.
• Travel throughout the City of Chattanooga to meet clients in their homes, at partner sites, and in the community.
• Develop individualized stability plans addressing needed home repairs and connect clients to supportive services.
• Track all communications and referrals in case files; provide regular updates to the Program Manager and Habitat for Humanity partners.
• Collaborate with Critical Home Repair team and homeowners to address issues before a repair partnership moves forward.
