Job summary

Habitat for Humanity Chattanooga Area is seeking an Elder Housing Liaison to provide intake, assessment, and wrap-around social services for clients. The role travels across Chattanooga to meet clients at homes, partner sites, and community locations, coordinating with the Critical Home Repair team to design stability plans and connect clients to supportive resources.

Qualifications

• > Bachelor’s degree in social work, human services, sociology, psychology or related field from accredited university required OR equivalent and relevant experience of 4 years or greater.
• > Minimum of two (2) years of experience providing social services, case management, or client intake and assessment to individuals, older adults, and families required.
• > Experience with housing support, home repair, homeownership, or financial stability programs preferred but not required.

Responsibilities

• Provide intake, assessment, and social services to individuals and families.
• Develop individualized stability plans addressing home repairs and connect clients to supportive services.
• Collaborate with Critical Home Repair team and partner programs; track referrals in case files.
• Travel throughout Chattanooga to meet clients in homes, partner sites, and community settings.
• Maintain professional relationships with Habitat for Humanity staff and community organizations.
• Attend meetings and training; accommodate client schedules including evenings/weekends.
