Job summary

Preferred Family Healthcare is seeking a Housing Liaison in Kirksville, MO to assist individuals with disabilities or in recovery in securing safe, affordable housing. You will work within the Housing Department to resolve housing crises and connect clients with local resources.

You will collaborate with Continuums of Care, support housing stability, and engage landlords while promoting access to community services and benefits.

Qualifications

• Bachelor’s in Social Work, Psychology, Sociology or related case management experience in housing.
• AS in Behavioral Health Support from an approved institution eligible to provide community support services.
• Qualified to provide Community Support through specified educational background and experience as defined in community-support qualifications.

Responsibilities

• Practice Housing First, harm reduction, and low-barrier service models when working with individuals or households.
• Attend Coordinated Entry/Case Conferencing meetings within the local Continuum of Care.
• Reach out to households currently not engaged in CPR or CSTAR services experiencing a housing crisis or homelessness.
• Refer and/or transport individuals to housing and related services for needs including behavioral health, substance use, physical health, employment, etc.
• Build and strengthen relationships with local landlords and educate on community programs and referrals for their units.
