Under general supervision of the Stability Services Director, the Elder Housing Liaison provides intake, assessment, and social services to individuals and families served on behalf of Habitat for Humanity. The Liaison travels throughout the City of Chattanooga to meet clients in their homes, at partner sites, and in the community to complete intakes, gather and verify required documentation, assess needs, and determine next steps. The Liaison will be responsible for working collaboratively with the Critical Home Repair team and clients to develop individualized stability plans that address needed home repairs and connect clients to supportive services, making resource referrals as necessary to other internal or external partner programs, tracking all communications and referrals in case files, and providing regular updates to the Program Manager and Habitat for Humanity partners. The Liaison participates in Critical Home Repair intakes whenever possible and also works with homeowners who opt in or are referred by staff when issues must be addressed before a repair partnership can move forward. These include hoarding, situations in which a homeowner may no longer be able to live safely alone, homes that may not be realistically repairable, and needs for area resources, including legal and estate planning support. The goal of this role is to provide wrap‑around services that complement home repairs, helping clients remove barriers, build stability, and remain safely housed.
