National Church Residences in Cleveland is seeking a part-time Service Coordinator to support senior residents at Colman Court. The role offers competitive pay and excellent benefits, with schedule Monday–Friday, 20 hours per week, no evenings or weekends.

You will advocate for residents, coordinate programs, liaise with community services, and help residents access meals, transportation, housekeeping, and other supports.

Qualifikationen

• Bachelor's degree in social work or related field preferred.
• Two or more years of experience delivering social services to elderly and/or family populations preferred.
• Working knowledge of supportive services and local community resources.
• Demonstrated ability to advocate, organize, problem-solve, and deliver measurable results for residents.

• Advocate, organize, problem-solve, and deliver results for residents.
• Coordinate programs and activities for residents.
• Liaise with community agencies and networks to secure services for residents.
• Identify low-cost service providers and negotiate discounts.
• Engage residents to identify needs and refer to community services.
• Develop a Resource Directory of state and local providers.
• Sponsor educational events on health care, agency support, life skills and referrals.
• Provide quality customer service to residents and address concerns.
• Assist residents in accessing services such as housekeeping, meals, transportation, personal services, and day care counseling.
• Monitor and relay feedback from residents and families.
• Manage relationships internally and externally with effective communication.
