Job summary

KLEO Community Life Center seeks a Housing and Case Management Supervisor to lead the adult single-resident program at ELEVATION House. The role oversees case management, housing navigation, and supportive services within a trauma‑informed, harm‑reduction, and Housing First framework.

You will coach Youth Client Services Specialists, drive housing stability outcomes, and build community collaborations to transition residents from shelter to permanent housing.

Qualifications

• Bachelor’s degree in Social Work or related field required; MSW preferred.
• Minimum of 3 years direct case management experience in housing, homelessness services, or supportive social services.
• Minimum of 1 year of supervisory, team lead, or clinical oversight experience.
• Strong grounding in Housing First principles, Harm Reduction, Trauma-Informed Care, and adult service delivery systems.
• Proficiency in HMIS and electronic documentation tools.

Responsibilities

• Directly supervise, mentor, and evaluate Youth Client Services Specialists assigned to the adult single-resident program at ELEVATION House.
• Conduct regular supervision sessions to review case progress, audit service delivery, and guide interventions for complex adult client barriers.
• Maintain a reduced direct caseload of approximately 10–12 residents.
• Oversee intakes, assessments, and housing stabilization plans by direct reports.
• Ensure progress notes, service plans, incident documentation, and exit records are accurately maintained in HMIS within 24–48 hours.
