Job summary

THRIVE in Kentucky seeks a Senior Housing Case Manager to lead housing programs, manage data, and train staff. You will monitor daily operations, supervise schedules, and ensure documentation and compliance standards are met.

The role includes caseload management, developing individualized housing plans, coordinating services, and representing THRIVE in the community. You will also cover staff absences, report concerns to leadership, and collaborate with housing leadership to maintain

Qualifications

• Bachelor’s degree in a related field plus two years’ relevant experience.
• RN license with BA and two years’ related experience.
• CASAC credential with two years’ related experience.
• Bachelor’s degree or higher with five years’ direct experience with behavioral health or related fields; Master’s may substitute for up to one year.

Responsibilities

• Meet monthly with participants to develop individualized housing plans.
• Coordinate services and link to community supports to avoid homelessness.
• Maintain electronic health record documentation per THRIVE policies.
• Provide guidance and training to other case managers.
• Oversee staff schedules, timecards, and annual reviews.
• Represent THRIVE at community meetings and ensure program compliance.
