THRIVE is seeking a Senior Housing Case Manager to lead housing programs, manage data, and train staff. The role supports daily operations, monitors schedules, and ensures compliance while maintaining client-centered housing plans and community linkages.

The position involves coverage during absences, representing THRIVE in the community, and reporting concerns to leadership. supervisory duties are included.

Qualifications

• Bachelor’s degree in a related field and two years’ direct service experience.
• NYS licensure and current RN registration with a bachelor’s degree and two years’ of relevant experience.
• A CASAC credential with two years’ of related experience.
• Advanced degree may substitute for up to one year of experience.

Responsabilités

• Assist with data collection and maintenance in electronic systems (evolve, CAIRS, HMIS).
• Train new housing case managers and facilitate staff meetings.
• Develop individualized housing plans and coordinate services for clients.
• Ensure documentation and compliance per THRIVE policies and oversight agency.
