Qualifications

• Bachelor’s degree in a field related to housing, social services, or health care and two years’ experience in direct services or linking clients to community resources.
• RN with BSN or related licensure and two years’ experience in behavioral health or housing services.
• CASAC credential with two years’ applicable experience.
• Bachelor’s degree with five years’ direct experience with individuals with behavioral health needs; or Master’s as substitute for up to one year of experience.

Responsibilities

• Meet monthly with each program participant on caseload to provide housing case coordination services.
• Develop written housing support plans based on strengths and individualized needs.
• Link recipients to services and supports to avoid homelessness.
• Coordinate treatment with providers, as necessary.
• Maintain electronic health records in THRIVE systems.
• Monitor and maintain high standard of service delivery.
• Represent THRIVE at community meetings and report key issues to leadership.
