Job summary

Kleo Community Life Center is seeking a Client Services Specialist to provide person-centered, trauma-informed case management for single adults experiencing homelessness at Elevation House in Chicago. The role focuses on reducing housing barriers, stabilizing residents, and connecting them to vital resources.

Responsibilities include comprehensive intake, housing stability planning, benefit acquisition, and timely documentation to meet grant and Medicaid requirements.

Qualifications

• Bachelor’s degree in Social Services or related behavioral science field preferred.
• Minimum of 3 years of case management experience with unhoused adults or behavioral health needs.
• Lived experience or shared cultural background with residents is valued.

Responsibilities

• Engage residents with low-barrier, trauma-informed case management and housing stability planning.
• Perform comprehensive intake and assessments within 24 hours of entry.
• Develop Individual Housing Stability Plans (IHSP) using SMART goals.
• Coordinate housing placement options (RRH, PSH, vouchers, private market).
• Maintain timely documentation for grant funding and compliance.
