Gentiva Hospice in Chattanooga is seeking a Patient Care Manager to lead the hospice team, directing clinical operations, coordinating care, and developing staff to uphold superior patient care and regulatory compliance.

The role requires an RN licensed in Tennessee with 3+ years of direct patient care, including hospice or home health, and a proven ability to lead a diverse care team in a fast-paced environment.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years of direct patient care experience, including hospice or home-based care.
• Demonstrated ability to apply hospice principles, industry standards, and regulatory compliance (Medicare, Medicaid, JCAHO, ACHC).
• Knowledge and compassion for terminally ill patients and their families.

Responsabilités

• Oversee and direct patient care services to ensure high-quality hospice care.
• Coordinate visit scheduling and clinical prioritization based on medical need.
• Triage incoming referrals and clinical concerns promptly and compassionately.
• Lead IDG meetings, present patient updates, coordinate care plans, and document minutes.
• Mentor and train associates, supporting development, performance, and compliance.
• Ensure care delivery meets regulatory standards and company policies.
• Support performance improvement initiatives and operational efficiencies.
