Gentiva Hospice seeks a Patient Care Manager to oversee clinical operations, coordinate interdisciplinary teams, and ensure compliant, high-quality hospice services.

The role includes directing patient care, managing visits, triaging referrals, leading IDG meetings, mentoring staff, and enforcing regulatory standards while supporting performance improvement at the Clayton, NC location.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years direct patient care experience, including 2 years in a hospice or home-based care setting.
• Demonstrated knowledge of Medicare/Medicaid/JCAHO/ACHC regulatory standards.
• Ability to lead a diverse care team and adapt to varied patient needs.

Responsabilités

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and prioritize visits based on clinical judgment.
• Triage incoming referrals and medical concerns promptly and compassionately.
• Lead Interdisciplinary Group (IDG) meetings and document outcomes.
• Mentor and support associates to improve performance and compliance.
• Ensure all care delivery meets regulatory standards and company policies.
• Support performance improvement initiatives and operational efficiencies.
