Gentiva Hospice seeks a dedicated Patient Care Manager to lead clinical excellence and ensure compliant hospice services. You will oversee patient care delivery, coordinate care plans, and supervise staff to uphold quality standards.

Reporting to the Executive Director or Administrator, you will manage scheduling, triage referrals, mentor associates, and drive regulatory compliance and performance improvement across the location.

Qualifications

• Must hold RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum 3 years of direct patient care, including 2 years in hospice/home-based care.
• Knowledge of hospice principles, regulatory standards (Medicare/Medicaid, JCAHO, ACHC).
• Compassion for terminally ill patients and families; ability to lead diverse teams.
• Strong interpersonal, organizational and leadership abilities; documentation and tech proficiency.

Responsabilités

• Oversee patient care services to ensure high-quality hospice delivery.
• Manage visit scheduling and prioritize based on clinical judgment.
• Triage referrals and clinical concerns promptly and compassionately.
• Lead or facilitate Interdisciplinary Group (IDG) meetings, document minutes and coordinate care plans.
• Mentor and train associates; support performance and compliance.
• Ensure care meets all regulatory and company standards; participate in quality initiatives.
