Job summary

Three Oaks Hospice seeks a Director of Clinical Operations RN to provide strategic leadership, operational oversight, and clinical guidance to deliver high-quality hospice care across the agency. You will supervise clinical and non-clinical teams, optimize staffing, and drive regulatory adherence.

You will partner with medical directors and senior leadership to ensure accurate documentation, timely certifications, and patient-centered care while maintaining survey readiness and budget alignment.

Qualifications

• BSN is required and RN license must be active.
• Master’s degree in nursing, healthcare administration, or related field preferred.
• Minimum of 2 years of clinical management or supervisory experience in hospice or home health.
• Strong knowledge of hospice philosophy and end-of-life care.
• Experience with EMR systems; Homecare Homebase strongly preferred.

Responsibilities

• Direct the overall clinical operations of the assigned team(s) ensuring standardization of processes and regulatory compliance.
• Supervise and support RN, LPN/LVN, CNA, SW, Chaplains, Team Managers, and IDG members.
• Ensure consistent implementation of clinical policies and evidence-based practices across teams.
• Ensure timely hospice admissions by streamlining workflows with intake and clinical teams.
• Lead QAPI programs and performance improvement projects.
