Job summary

Three Oaks Hospice, Inc. seeks a dedicated Registered Nurse to provide skilled nursing care in patients' homes. You will coordinate with the interdisciplinary team, patients, families, and referring agencies to deliver compassionate hospice and palliative care.

The role includes on-call responsibilities, assessment, care planning, and education to support patients and families facing end-of-life needs.

Qualifications

Graduate of an accredited nursing program.
Current state RN license and driver's license.
Two years' RN experience in clinical care; hospice/home health preferred.
CPR certification.

Responsibilities

Perform initial and ongoing comprehensive assessments of patients in their home, addressing physical, functional, psychosocial, and environmental needs.
Implement and revise the individualized plan of care (POC) with the care team.
Educate patients and families about disease, end-of-life care, and self-care techniques; train staff as needed.
Provide preventive and rehabilitative nursing procedures.
Document progress toward goals in clinical notes.
Coordinate services with the inter-disciplinary team and referring agencies.
Initiate appropriate case management and referrals as needed.
Communicate changes in patient needs to physicians and the care team.
Determine service scope and frequency based on acuity and caregiver ability.
Assess caregiver ability and support needs.
Use current processes and technology for communication.
Determine admission and re-certification criteria for hospice care.
Provide on-call coverage as assigned.
