Three Oaks Hospice, together with Agape Hospice Care and other sister organizations, is seeking a Director of Clinical Operations (RN) to provide strategic leadership across clinical services. The role emphasizes regulatory compliance, staffing optimization, and performance improvement to deliver high-quality hospice care.

The DCO will oversee clinical and non-clinical teams, partner with medical leadership, manage budgets, and ensure survey readiness.

Formación

• BSN required; master's preferred.
• Active RN license and current practice knowledge.
• Minimum 2 years in hospice or home health management.

Responsabilidades

• Direct the overall clinical operations of the assigned team(s), ensuring standardization of processes, outcomes, and regulatory compliance.
• Supervise and support RN, LPN/LVN, CNA, SW, CH, TM, and IDG members.
• Ensure consistent implementation of clinical policies, procedures, and evidence-based practices.
• Ensure timely hospice admissions by streamlining workflows with intake and clinical teams.
• Partner with Medical Directors and attending physicians for accurate documentation and timely certifications.
• Lead IDG meetings and participate in QAPI programs and PIPs.
• Audit clinical documentation for accuracy and compliance, addressing deficiencies promptly.
• Monitor KPIs and drive performance improvement initiatives across patient care and regulatory adherence.
