Gentiva Hospice is seeking a Patient Care Manager to lead clinical operations and oversee patient care delivery in a high-quality, compliant hospice program. You will coordinate care, schedule visits, and mentor staff to uphold patient-centric, regulatory standards.

The role requires an RN license, 3+ years in direct patient care with hospice/home health experience, and strong leadership and documentation skills. On-site location with comprehensive benefits offered.

Qualifications

• RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years direct patient care, including 2 years in hospice or home-based care.
• Knowledge of Medicare, Medicaid, JCAHO, ACHC regulatory requirements.
• Compassion for terminally ill patients and families.
• Ability to lead a diverse care team and adapt to varied needs.
• Strong interpersonal, organizational, and leadership skills.
• Proficiency in clinical documentation and technology systems.

Responsibilities

• Oversee and direct patient care to ensure high-quality hospice services.
• Manage visit scheduling and prioritise based on clinical judgment.
• Triage referrals and clinical concerns promptly and compassionately.
• Lead or facilitate IDG meetings, present updates, coordinate care plans, document minutes.
• Mentor and train associates to support compliance and performance.
• Ensure care delivery meets federal, state, local regulations and company policies.
• Support quality improvement and operational efficiency within the location.
