Gentiva Hospice seeks a Patient Care Manager to lead clinical operations, coordinate care, and develop staff for compliant, high‑quality hospice services. You will oversee patient care, triage referrals, and guide IDG meetings while ensuring regulatory standards are met.

Ideal candidates bring an RN license, 3+ years direct care (2+ in hospice), and strong leadership and documentation skills. Emphasis on compassionate, compliant care across diverse teams.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years direct patient care, incl 2 in hospice/home-based care.
• Demonstrated ability to apply hospice principles and regulatory compliance.
• Compassion for terminally ill patients and families.
• Ability to lead a diverse care team with strong communication.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and clinical prioritization.
• Triage referrals and medical concerns promptly and compassionately.
• Lead or facilitate IDG meetings and document minutes.
• Mentor and train associates in performance and compliance.
• Ensure care delivery meets regulatory standards and policies.
• Support performance improvement and operational efficiencies at the location.
