Zusammenfassung

Stella Maris Inc. seeks a Home Hospice Clinical Manager (RN) to lead our expanding Home Hospice program. You will guide field nurses and clinicians, develop care pathways, and ensure high-quality patient care while collaborating with leadership.

The role requires an Maryland RN license, hospice experience, and leadership skills to mentor staff and improve clinical outcomes across the program.

Qualifikationen

• RN license in Maryland (BSN preferred).
• Prior hospice experience required (3–5 years).
• CHPN certification strongly preferred.
• Leadership and prior home health experience preferred.

Aufgaben

• Provide leadership and guidance to the Home Hospice staff.
• Develop, coordinate, direct, and evaluate all clinical services for the program.
• Hire, supervise, evaluate, and mentor staff; deliver staff evaluations.
• Conduct patient visits; support nursing staff, patients and families.
• Consult with administration on policy formulation consistent with standards.
• Oversee new intake referrals and develop clinical care pathways.
• Participate in QAPI plans; oversee quality programs and utilization review.
• Measure quality of care and drive initiatives for improvement against industry standards.
• Coordinate onboarding and ongoing staff development with Educator.
