Stella Maris Inc. in Maryland is seeking a Home Hospice Clinical Manager (RN) to lead our Home Hospice program, mentor field nurses and clinicians, and drive clinical excellence.

The successful candidate will oversee staff hiring, coaching, patient visits, policy guidance, and quality initiatives, partnering with administration to ensure safe, compassionate care and regulatory compliance. Qualifications

• Leadership required to guide hospice staff and programs.
• Experience in hospice care and clinical leadership a must.
• Ability to mentor field nurses and clinicians toward high-quality patient care.

Responsibilities

• Lead and mentor Home Hospice staff.
• Develop, coordinate, and evaluate clinical services for the Home Hospice program.
• Conduct patient visits and support nursing staff, patients, and families.
• Consult with administrator on policy and regulatory standards.
• Oversee new intake referrals and develop clinical care pathways.
• Participate in QAPI plans and quality initiatives.
• Monitor quality measures and staff performance; conduct evaluations.
• Collaborate with educator for onboarding and staff development.
