Job summary

Stella Maris is seeking a Home Hospice Clinical Manager (RN) to lead and mentor our Home Hospice staff, develop clinical pathways, ensure high-quality patient care, and advance program growth in Maryland.

The role requires RN licensure in Maryland, hospice leadership experience, and CHPN certification preferred; leadership and collaboration with administration are essential to drive QAPI, staffing, and service excellence.

Qualifications

• RN license in Maryland (BSN preferred).
• 3-5 years hospice experience preferred.
• CHPN certification strongly preferred.
• Leadership and prior home health experience preferred.

Responsibilities

• Hires, supervises, evaluates, and mentors staff assigned to their team.
• Prepares and delivers staff evaluations in a timely manner.
• Visits patients to support nursing staff, patients and families; acts as resource to address and resolve issues.
• Consults with administrator on policy formulation ensuring alignment with standards and regulations.
• Oversees intake referrals for the hospice program.
• Develops clinical care pathways using evidence-based practice to improve outcomes and safety.
• Participates in QAPI plans and oversees quality initiatives.
