Job summary

Peoples Hospice and Palliative Care seeks a Clinical Manager to lead and supervise hospice services delivered by the interdisciplinary team in homecare and facility settings. You will oversee case referrals, coordinate with the Medical Director, and ensure care plans meet quality standards while advancing organizational goals.

Strong nursing background required with prior hospice and supervisory experience preferred.

Qualifications

• Bachelor's in nursing preferred.
• A minimum of 2 years’ RN experience is preferred.
• At least 1 year of hospice experience.
• A minimum of 1 year supervisor experience is preferred.

Responsibilities

• Serves as a member of the leadership team for Peoples Hospice division.
• Assumes overall responsibility for oversight and management of the clinical services provided by the interdisciplinary team (IDT) within the homecare and facility setting.
• Receives case referrals. Reviews patient information to determine hospice/home care needs and assigns appropriate personnel.
• Reviews and evaluates each case to promote effective performance and quality home care services; guides clinicians.
• Conducts supervisory visits to observe clinical performance of assigned team members.
• Reviews patient diagnoses, prognosis, medications, procedures and clinical course; assists in setting goals and care plans.
• Attends case conference meetings to coordinate care and discuss IDT involvement.
• Conducts quarterly record reviews and communicates findings to leadership and staff.
