Job summary

Optum Home & Community Care is expanding its Senior Community Care team to provide in-home nursing support for patients in skilled facilities and senior housing. The role emphasizes care coordination, documentation, and collaboration with the care team to ensure comprehensive patient care.

The position requires an active RN license, BLS certification, and at least 3 years of clinical experience, with the ability to lift 30 pounds and access reliable transportation.

Qualifications

• Active RN license in the state of residence or able to obtain by start date.
• BLS certification is required.
• 3+ years of clinical practice experience.
• Ability to lift up to 30 pounds and perform physical assessments.
• Reliable transportation is required.

Responsibilities

• Ensure accurate and complete documentation.
• Collaborate with the interdisciplinary care team.
• Assist with care coordination activities and Medicare benefits determination.
• Support therapy coordination and facility-based meetings.
• Schedule and participate in family conferences and team presentations.
• Contribute to training, quality measures, and in-services.
