Job summary

Optum is seeking a Registered Nurse in Home Health to provide and direct nursing care in patients' homes, coordinating care with physicians and the care team. You will ensure continuity of care, participate in plan creation, and document per policy.

Primary responsibilities include supervision of staff, accurate assessments and coding, and effective communication with families, patients, and clinicians to drive quality outcomes. Competitive benefits are available.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Ability to coordinate total care and communicate with the physician and team.
• Strong documentation and adherence to policies and regulations.

Responsibilities

• Initiates, develops, implements, and revises the plan of care with the physician and team.
• Supervises home health aides and nurses, providing instruction and task assignment per regulations.
• Documents visits, performs OASIS assessments, and ensures timely coding and policy alignment.
• Communicates with patients, families, and providers to ensure quality home health care.
• Participates in on-call and weekend rotations and supports performance improvement.
