Job summary

Optum Home & Community Care, part of UnitedHealth Group, seeks a Senior Nurse Practitioner to deliver high-quality care at home, nursing homes, and assisted living facilities. You will perform assessments, develop plans of care, and coordinate with physicians and care teams to ensure seamless journeys for patients.

This role involves traveling between care sites to manage medical and behavioral conditions, document thoroughly, and participate in required trainings and scheduling.

Qualifications

• Certified Nurse Practitioner with national board certification.
• Graduate of an accredited MSN program and board certified (AANP/ANCC) with adult-gerontology or family nurse practitioner focus.
• Active and unrestricted license in the state of practice; DEA licensure or ability to obtain per state regulations.

Responsibilities

• Provide primary care delivery to assigned members, including chronic and acute conditions.
• Coordinate care across facilities, engage with providers, families, and care teams.
• Review plans of care, document findings, and ensure CMS-compliant records.
