Job summary

Optum is seeking a Registered Nurse in Home Health to provide and direct nursing care to patients in their homes per physician orders and agency policies.

The role involves coordinating the plan of care with the healthcare team, supervising home health aides, completing OASIS assessments, and ensuring quality care while maintaining compliance with state and federal regulations. A current RN license and valid driver's license are required.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver’s License, vehicle insurance, and access to dependable vehicle or public transportation.
• CPR Certification preferred or ability to complete within 90 days of hire (preferred).
• 1+ years of Home Health experience (preferred).
• Ability to work independently and strong written communication.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other healthcare professionals.
• Supervises care provided by home health aides and licensed nurses, assigns tasks per regulations, and provides required supervisory visits.
• Completes comprehensive assessments (OASIS) including medication reconciliation and documents visits per policy and payer requirements.
