UnitedHealth Group's Optum Home & Community Care seeks a registered nurse to coordinate in-home care for patients in nursing homes and assisted living. You will document, collaborate with the care team, and support therapists and providers, managing care plans and transitions.

Qualifications

Active and unrestricted RN license
Associate degree or higher in Nursing (RN)
Basic Life Support (BLS) certification
3+ years of clinical experience
Ability to lift 30 pounds and perform physical assessment
Access to reliable transportation

Responsibilities

Ensure accurate and complete documentation
Communicate and collaborate with the interdisciplinary care team
Assist the provider/team with various care coordination activities
Assist with Medicare Part A/B/D benefit determinations
Collaborate with APC and facility to respond to changes in condition
Assist in therapy coordination
Participate in facility-based meetings
Schedule and participate in family conferences and team meetings
Coordinate training, tracking, and compliance with quality measures
Create and conduct in-services for selected audiences
Assist in documentation, coding, medication reconciliation, nursing rounds, assessments
May write verbal orders in compliance with RN laws
Manage members during acute or sub-acute episodes
