Job summary

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Provides and directs nursing care to patients in their homes as prescribed by the physician and in compliance with laws, regulations, and agency policies.
• Coordinates total plan of care with other healthcare professionals and ensures continuity of patient care through communication with physician, agency staff, patient, family, and resources.
• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other healthcare professionals; supervises home health aides and licensed practical/vocational nurses.
• Performs comprehensive assessments (OASIS), medication reconciliation, documentation of visits, and adheres to plan of care and payer requirements.
• Participates in on-call and weekend rotation as needed and supports performance improvement and survey readiness.
