Job summary

Optum is seeking a Registered Nurse to provide and direct nursing care to patients in their homes, coordinating total plan of care with physicians and care teams, and ensuring compliance with laws and agency policies.

The role includes home visits, documentation, and collaboration with families and community resources. A valid RN license, CPR certification, and reliable transportation are required to perform duties in diverse home settings.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Valid driver's license and access to reliable transportation.
• Current CPR certification.
• Ability to work with diverse patients and families.

Responsibilities

• Provide high-quality clinical nursing services within scope of practice and infection control standards.
• Coordinate care with other members of the patient/client's care team from admission to discharge.
• Complete clinical nursing assessments per federal/state program requirements and payer needs.
• Ensure patient/client eligibility and medical necessity for services as defined by payer and agency policy.
• Develop and revise individualized plans of care/service plans with other community providers.
• Ensure plan of care frequency and duration meets patient needs and initiate revisions with physician approval.
