Qualifications
- Current BLS/CPR Certification.
- Valid CA Driver’s License.
- Understanding of illness/injury impact on development and behavior.
- Knowledge of state and federal home-care regulatory guidelines preferred.
- Experience with high-acuity patients a plus.
- Experience working with an interdisciplinary team highly desired.
- Case management fundamentals desired.
Responsibilities
- Perform evaluations in patients’ homes and develop individual care plans.
- Work with Physical Therapy Assistants to follow care plans.
- Help patients improve mobility and regain independence.
- Evaluate and report patients’ progress to medical directors.
- Supervise and mentor 2 PTAs, ensuring they meet performance standards.
