Job summary

Enhabit Home Health & Hospice is seeking a Nursing Leader to oversee coordination of home health services and ensure compliance with standards. The role involves collaboration with physicians, referrals, patients and caregivers to maintain seamless care delivery.

Required nursing degree with active license, 2+ years of recent nursing experience (including home health), CPR, and a valid driver's license. Bilingual (English/Spanish) is a plus; on-site attendance is essential.

Qualifications

Graduate of an approved school of professional nursing.
Valid clinical license per state requirements; compact license acceptable.
Minimum two years of nursing experience in the last 36 months.
At least one year of home health experience.
Knowledge of federal, state and local home care laws and regs.
CPR certification required.
Valid driver license and auto insurance required.
In-office attendance is essential (except for designated remote roles).
Preferred bilingual (English/Spanish).
Tech-savvy with MS Office and mobile devices.

Responsibilities

Coordinate home health services delivered to the patient per standards and procedures.
Review and approve patient information during start of care, recertification, resumption of care, or evaluation.
Facilitate relationships among physicians, referrals, patients, caregivers and staff.
Schedule patients to field clinicians and communicate with all parties to maintain coordination.
