Job summary

UnitedHealth Group, on behalf of Optum Home & Community Care, is seeking a Senior Advanced Practice Clinician to join the Senior Community Care program. You will deliver high-level care to members in home-based settings, including nursing homes and senior housing, performing assessments, care planning, and patient education.

This role emphasizes advanced practice in geriatrics; requires NP or PA certification, state licensure, and DEA where applicable.

Qualifications

Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
Current NCCPA certification and state licensure.
Active, unrestricted licensure in the practicing state.
Active and unrestricted driver's license with reliable transportation.
Current DEA licensure/prescriptive authority (where required).

Responsibilities

Conduct comprehensive assessments including medical history review, vitals, and physical exams.
Provide care coordination for high-risk patients in home and sub-acute settings.
Document accurately with ICD-10 coding and maintain records.
Collaborate with interdisciplinary care team and participate in advanced care planning.
