Job summary

UnitedHealth Group, through Optum Home & Community Care's Senior Community Care program, seeks an Advanced Practice Clinician (NP or PA) to deliver coordinated, patient-centered care for high-risk seniors in nursing homes, assisted living, and home settings.

The role involves comprehensive assessments, medication management, documentation, and collaboration with physicians and care teams to improve outcomes and quality of life. A $5,000 sign-on bonus is available for external hires.

Qualifications

• Current ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Current NCCPA certification and applicable state licensure.

Responsibilities

• Conduct comprehensive patient assessments including medical history, medication reconciliation and physical examinations.
• Support members during short-term skilled nursing and rehabilitation stays.
• Ensure timely clinical documentation including ICD-10 coding; collaborate with physicians and care teams.
• Facilitate advance care planning and educate patients and caregivers on plans.
