Job summary

UnitedHealth Group is seeking an Advanced Practice Clinician (NP/PA) to join the Senior Community Care (SCC) team. You will coordinate clinical care for high-risk members in nursing homes, senior housing, and assisted living settings, with a focus on integrated care delivery and improving access to care.

You will conduct assessments, manage sub-acute rehabilitation needs, document thoroughly, and collaborate with an interdisciplinary team to support patients and families.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Gerontological or Primary Care.
• Active, unrestricted licensure in the practicing state; ability to obtain by start date.
• Valid driver's license and access to reliable transportation.

Responsibilities

• Conduct comprehensive assessments including history, medication reconciliation, vital signs, and physical exams.
• Perform evidence-based screenings and point-of-care testing as appropriate.
• Support members during short-term sub-acute rehabilitation episodes.
• Ensure accurate and complete documentation, including ICD-10 coding.
• Communicate and collaborate with interdisciplinary care teams.
• Engage in advanced illness and end-of-life planning conversations.
• Provide counseling and education to patients and caregivers.
