Job summary

UnitedHealth Group is seeking an Advanced Practice Clinician (NP/PA) to join the Senior Community Care team. You will assess, plan, and coordinate care for high-risk members in nursing homes, senior housing, and home settings.

The role requires active certification (ANCC/AANP for NP or NCCPA for PA), active state licensure, and ability to obtain DEA where applicable. Competitive total compensation includes base pay and bonus, plus comprehensive benefits.

Qualifications

• Active ANCC or AANP certification for NP route
• Active NCCPA certification and state licensure for PA route
• Active and unrestricted state license where practicing
• DEA licensure/prescriptive authority where required

Responsibilities

• Conduct comprehensive assessments including med history, vitals and exams
• Coordinate care in sub-acute rehab settings
• Document accurately with ICD-10
• Collaborate with interdisciplinary care team
• Conduct advanced illness and end-of-life planning conversations
