Job summary

UnitedHealth Group seeks an Advanced Practice Clinician (NP/APC) to join the Senior Community Care team, delivering integrated care to high-risk members in long-term care settings. No on-call, weekends, or holidays required, with a $10,000 sign-on for external candidates.

The role emphasizes comprehensive assessments, care planning, and collaboration with an interdisciplinary team to improve patient outcomes across care settings.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Current, unrestricted licensure in the state you will be practicing in
• Active and unrestricted driver's license with reliable transportation
• Current, active DEA licensure/prescriptive authority or ability to obtain post-hire

Responsibilities

• Conduct comprehensive assessments including past medical history, medication reconciliation, vital signs and physical exam
• Support members during a short-term sub-acute rehabilitation episode
• Ensure accurate and complete documentation including ICD-10
• Communicate and collaborate with the interdisciplinary care team
• Conduct advanced illness and end-of-life discussions
• Provide patients and caregivers with counseling and education
