Qualifikationen

• Active ANCC or AANP certification for NP roles or NCCPA for PAs.
• Active, unrestricted state licensure and ability to obtain start date.
• Active and unrestricted driver’s license with reliable transportation.
• DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.

Aufgaben

• Conduct comprehensive assessments including medical history, medications, vitals and exams.
• Support members during short-term sub-acute rehabilitation episodes.
• Ensure accurate documentation including ICD-10 codes.
• Collaborate with interdisciplinary care team and perform advanced illness conversations.
• Provide counseling and education to patients and caregivers.
