Job summary
UnitedHealth Group is hiring for Senior Clinician roles within the Senior Community Care program to deliver clinical services in sub-acute, nursing home, and senior living settings. You will perform comprehensive assessments, coordinate care with an interdisciplinary team, and support patients and families in planning care.
Active NP or PA certification with licensure is required, along with DEA where applicable, and strong EMR skills.
Qualifications
- Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
- Current NCCPA certification and state licensure.
- Active, unrestricted state licensure where practicing and ability to obtain by start date.
- Active and unrestricted driver's license with reliable transportation.
- DEA licensure/prescriptive authority or ability to obtain post-hire (where required).
- Ability to form a collaborative practice agreement where applicable.
Responsibilities
- Conduct comprehensive assessments including medical history, med reconciliation, vitals and exam.
- Support members during short-term sub-acute rehabilitation episodes.
- Ensure accurate documentation including ICD-10 codes.
- Collaborate with interdisciplinary care team.
- Discuss advanced illness and end-of-life planning with patients.
- Provide counseling and education to patients and caregivers.
